Please review the following information before booking your appointment with Revitalize-Mobile IV+Wellness.
Virtual Consultation Requirements
For your safety and well-being, IV infusions and Injection services require a virtual appointment with our Physician yearly. Weight-Loss services require a virtual appointment every 3 months. Additional appointments may be added if changes in health conditions warrant. All Virtual appointments are included for no additional fees!
Pricing & Service Area
A $100 minimum applies to each appointment. An additional fee may apply if the service location is outside a 40-mile radius from Rhome TX. We will confirm all additional costs before your appointment.
No Payment will be taken until the time of service. A Credit Card is required for booking appointments and will not be charged unless the Cancelation Policy Fee is warranted. By booking an appointment you are agreeing with the Cancelation Policy. Appointments may be cancelled up to 6 hours before appointment. A $50 fee will be charged for cancellations within 6 hours of appointment time.
